The report examines the violence in James' family and how child protection, police, health and education authorities responded to the family and to James in particular. The report highlights agency failings, particularly in relation to interagency communication and collaboration, which were viewed as significant contributors to James' death. A range of recommendations are made for the improvement of interagency communication and the necessity for active involvement of family/whanau in child welfare decisions.
