Failings by Child, Youth and Family Services, the Police, schools and other professionals were identified as contributing to and maintaining family violence in the lives of Saliel and Olympia. Lack of interagency communication is emphasised as a significant factor undermining the care and protection of the sisters. Recommendations for improving interagency collaboration in instances of child abuse and neglect are made.
