N2  - This report presents the results of the independent inquiry into the circumstances surrounding the death of a two year old child in New Zealand, the daughter of M and Simi Tali. The child was under the legal supervision of the Department of Social Welfare when she was killed by her mother, after an extended period of abuse, in July 1987. The report describes the facts of the case and examines the appropriateness and adequacy of services and resources in terms of what was provided and suggested practice. The authors' conclusions and recommendations address: the interests of the child and the priority of child safety, situational analysis replacing the 'disease model' that focuses on the caregiver, a multidisciplinary approach including the relationships between Departmental social workers and others with an interest in child protection, cultural appropriateness, monitoring of staff and staffing levels, training, codes of practice, and research into child abuse in a New Zealand context
