Summary: In this report, the authors discuss the findings of a review into the deaths of 12 children in the care of Children, Young Persons and their Families Services during 1994 and 1995. The authors discuss the age, ethnicity, gender, number of siblings, family situation and behavioural issues of the children who had died. Five of the 12 deaths were from non-accidental injury, three were accidental, three were suicide, and one was natural. The report concludes that Children, Young Persons and the Families Services failed to prevent some of these deaths, and the authors call for more accountability of those who cause lethal injuries to children.
